Coordinated Family Support (CFS) Services

To be referred to APTA's CFS, please contact your WRC service coordinator.

APTA's CFS program is tailored for adults with developmental disabilities who choose to live with their family.  CFS is adaptable to cater to the needs of our clients and encourage the maintenance of our clients' family unit.  

Description of APTA's Coordinated Family Support Services:

Preservation of Family Unit

APTA offers assistance in facilitating the identification and provision of needed support for clients to effectively reside in their own familial household.    

Identification of Appropriate Support

APTA will provide training and guidance to the clients and their family in navigating a diverse range of services and supports tailored to their specific needs, including strategies for overcoming barriers to accessing generic services and regional center resources.  

Nature of Disability

APTA will offer training and support to further the knowledge and resources needed related to our clients' developmental disability diagnosis or diagnoses, its etiology, and the various forms of support that are accessible to meet their disability needs. 

Closing Service Gaps

APTA will provide assistance to organize and coordinate services and support to provide continuity of training among providers, in accordance with the needs of our client and their family.  

Health/Medical

APTA will assist our clients in organizing service delivery, including medical and other necessary appointments, including access to urgent and/or emergency services. 

Transportation Support

APTA will assist in assessing the various options for transportation modalities that are safe and available for our clients. E.g. Access membership, LA City Ride, Metro Transit, and Driving Education. 

Identification of Back-Up Support

APTA will assist our clients and their families to identify substitute providers and services by devising an effective plan for backup support. Our team can offer temporary direct care services up to 90 days. 

Support Planning for Aging Caregivers

APTA will provide comprehensive and long-term planning tailored specifically for aging clients and whose family and natural support systems are being impacted by the process of aging as well. 

Fostering of Independence

APTA will provide our clients with training that is specifically designed to promote their independence, setting them up for success in their future endeavors.   

Linguistic and Cultural Diversity

APTA's CFS will offer a variety of training approaches that align with the language preferences of our clients and their families.
