APTA offers state-wide independent facilitation services to clients of regional centers ranging across all age groups (children and adults). We have expertise in creating a comprehensive person-centered plan and assisting our clients in selecting inclusive and integrated vendor providers that can effectively fulfill their individual needs. We are highly skilled in creating a spending plan that will successfully meet the wants and needs of our clients.
APTA's independent facilitation services aim to identify and address both immediate and long-term needs, develop potential solutions to meet those needs, actively participate and advocate for the clients during the person-centered planning process, and participate in developing the Individual Program Plan (IPP).
The person-centered planning approach offered by APTA assists clients in developing and articulating a future vision, assessing various options, engaging in decision-making, resolving problems, monitoring progress, and swiftly implementing necessary adjustments. It highlights the importance of taking personal responsibility, which includes making thoughtful judgments such as assessing the need for obtaining appropriate support.
Conducting person-centered planning meeting and creating a person-centered plan (PCP) report that includes our clients' current needs, unmet needs, and goals.
Engagement in the development of Individual Program Plan (IPP) meetings to accurately reflect the process of transitioning towards self-determination.
Advocacy is provided to ensure that the clients' budget is aligned with their current needs and unmet needs identified in the PCP report.
Creating a comprehensive spending plan that accurately represents our clients' desired service providers and the specific services they require.
Assistance in accessing an appropriate financial management services (FMS) agency.
Assistance in locating and enrolling the client's preferred inclusive service providers to the FMS agency.
Assistance in ensuring an efficient and smooth self-determination program service delivery transition, by helping our clients navigate the intricate process.
We will provide ongoing independent facilitation services upon the request of our clients and their families. APTA's ongoing independent facilitation services will be funded through our clients’ approved SDP spending plan.
We will ensure that our clients and their families receive timely updates regarding any new directives and changes implemented by the Department of Developmental Services (DDS) pertaining to the self-determination program.
